Please read the following very carefully, and check all corresponding boxes to acknowledge your agreement.

I, the undersigned participant (and/or parent or legal guardian of the participant if under 18), understand and acknowledge that trampoline gymnastics involves inherent risks, including but not limited to: falls, collisions, awkward landings, equipment failure, and other accidents that can result in severe injury, paralysis, or death. I voluntarily choose to participate (or allow my child to participate) in activities organized, supervised, or sponsored by Free Solo Trampoline, LLC.

I understand that even with appropriate safety measures, supervision, and coaching, serious injury or death can occur. I knowingly and freely assume all such risks, both known and unknown, and take full responsibility for my participation.

In consideration for being allowed to participate in any activity, training session, competition, or event associated with Free Solo Trampoline, LLC, I hereby release, discharge, and agree to hold harmless:

Free Solo Trampoline, LLC, its owners, officers, staff, coaches, volunteers, independent contractors, members, athletes, parents, guardians, agents, sponsors, successors, and assigns (collectively “Released Parties”) from any and all liability, claims, demands, actions, or causes of action of any kind arising from or related to participation in such activities, including but not limited to those caused by the negligence or gross negligence of the Released Parties.

This release extends to all claims of any kind, including property damage, bodily injury, or wrongful death, whether occurring before, during, or after participation.

I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, damages, losses, or expenses (including attorney’s fees) arising from my (or my child’s) participation, whether caused by negligence or otherwise, including claims brought by third parties.

In the event of injury, accident, or illness, I hereby grant permission to Free Solo Trampoline, LLC staff, coaches, or volunteers to seek emergency medical treatment for myself or my child, as deemed necessary. I authorize any licensed physician, hospital, or medical personnel to provide such medical services as may be required. I understand that I am solely responsible for any costs associated with such medical care.

I consent to the use of photographs, videos, or other media recordings taken during activities for promotional or educational purposes by Free Solo Trampoline, LLC without compensation.

I agree that if any portion of this agreement is held invalid or unenforceable, the remaining portions shall continue in full force and effect. This agreement shall be governed by and construed in accordance with the laws of the State of Colorado.

I have read and fully understand this waiver and release of liability. I am aware that by signing this document, I am waiving substantial legal rights, including the right to sue Free Solo Trampoline, LLC or any Released Party. I sign this document freely and voluntarily.
